Intraoperative Neuromonitoring (IONM)

What is Neuromonitoring?

Neuromonitoring, also known as Intraoperative Neuromonitoring (IONM), is a specialized medical service used during surgical procedures to observe the function of the nervous system in real time. It acts as a safety mechanism, allowing surgeons to detect and prevent potential nerve damage before it becomes permanent.

This is especially important in surgeries where the brain, spinal cord, or peripheral nerves may be at risk—such as spine surgeries, brain tumor removals, or procedures involving the head, neck, and major nerves.

How It Works

Using advanced equipment and neurophysiological expertise, neuromonitoring tracks electrical signals in the nervous system throughout surgery. The information is continuously analyzed and communicated to the surgical team, enabling immediate action if changes in neural function are detected.

This live feedback significantly lowers the risk of postoperative complications such as loss of movement, sensation, hearing, or other neurological functions.

IONM Modalities

Somatosensory Evoked Potential (SSEPs)

SSEPs measure the electrical activity of sensory pathways in response to external stimuli, typically electrical stimulation applied to peripheral nerves. Changes in SSEPs can indicate potential damage to the spinal cord or peripheral nerves during surgery

Motor Evoked Potentials (MEPs)

MEPs assess the integrity of motor pathways by eliciting electrical responses in muscles following direct cortical or spinal cord stimulation. Changes in MEPs can signal potential damage to the motor cortex or spinal cord.

lectroencephalography (EEG)

EEG monitors the electrical activity of the brain by placing electrodes on the scalp. Intraoperative EEG helps assess cerebral function and detect abnormalities that may arise during surgery, such as ischemia or seizures.

Brainstem Auditory Evoked Potentials (BAEPs)

BAEPs measure electrical responses in the auditory pathway following auditory stimuli. They are particularly useful in monitoring procedures near the brainstem, helping identify potential damage to auditory and brainstem functions.

Facial Nerve Monitoring

In surgeries involving the facial nerve, such as those addressing tumors or acoustic neuromas, parotidectomy and maxillofacial procedures, facial nerve monitoring involves measuring muscle responses to stimulation of the facial nerve. This helps prevent inadvertent such as facial paralysis during such surgeries.

Train-of-Four (TOF)

TOF is used to assess neuromuscular transmission when neuromuscular blocking agents (NMBA) are given to block musculoskeletal activity. By assessing the depth of neuromuscular blockade, peripheral nerve stimulation signal transmission will not be affected and this will provide reliable neurological data for the patient.

Direct Electrical Cortical Stimulation(DECS) / Cortical Mapping

Cortical mapping is a diagnostic procedure that involves the identification and localization of functional areas within the cerebral cortex by the use a special probe (monopolar or bipolar) which discharges small stimulation current as we observe functional areas of the brain . This process is crucial for understanding brain functions and planning neurosurgical interventions, such as tumor resection or epilepsy surgery.

Direct Epidural Waves (D-Waves)

D-wave (Direct waves) are Motor Evoked Potentials (MEPs) generated by a single transcranial stimulation and captured by attaching an epidural recording electrode caudal to the vulnerable area. D-wave recordings, in conjunction with other modalities such as MEPs and SSEPs, provide a comprehensive assessment of spinal pathway functioning during surgery especially in spinal cord tumor resection.

Electromyography (EMG)

EMG records the electrical activity of muscles. In neuromonitoring, it is often used to assess the function of peripheral nerves and muscles during surgeries that involve nerve manipulation, such as spinal or peripheral nerve surgeries. In electromyography, we have two types: Free-run EMG is defined as continuous monitoring performed throughout the surgery. Stimulus-triggered EMG is frequently used to aid placement of pedicle screws and involves the use of a handheld monopolar probe controlled by the surgeon. Stimulus-triggered EMG assesses pedicle integrity (i.e., if a screw has breached the pedicle wall posing a risk to the nerve root). Both techniques can be used to monitor lumbar, thoracic and cervical fusion procedures, in addition to cranial nerve function to detect nerve root injury.
